Four places initially, though they might add more later in an expansion or something. You have to pick and choose which ones are the most awesome hehe.

I'm assuming both shoulders, but that leaves 2 slots.
Nope not sure where exactly, I just heard there were four slots total.
Hmm some where I heard 12-14 slots.

I think you get trophies from doing certain things/quest and you can place the trophy some where on your armor, not sure about how many places there are to put them.
